INITIAL COMMENTS
 
This report is a result of incident follow-up investigation conducted on October 19, 2016 by staff from the Division of Accountabilty and Program Improvement. Based on the findings of the investigation, Clem-Mar House Inc. handled the incident appropritely. Therefore, the facility was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
