This report is a result of an on-site complaint investigation conducted on August 25, 2008 and August 26, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site complaint investigation, the allegations made against - Clem-Mar House, Inc. were unsubstantiated.
