This report is the result of a complaint inspection conducted on January 31, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, Clem-Mar House, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The allegations were partially substantiated and the following deficiency was identified during the inspection. |

The residential facility shall:
(1) Be free of rodent and insect infestation.

Observations Based on interviews, records and a physical plant inspection, the facility failed to keep the facility free of insects.
Findings:
On January 31, 2012 between 09:30 AM and 11:30 AM four residents, three staff and one pest control expert were interviewed. Residents were asked if the facility was free of insects. Residents 1, 2, 3, and 4 indicated the facility had bed bugs.
Three of three staff interviewed on January 31, 2012 indicated reports of bed bugs had been referred to the pest control contractor. A physical plant inspection was conducted in the company of the pest control contractor. No evidence of bed bugs was found in the bedrooms located on the second floor of the facility. A review of the last eight service invoices indicated bed bugs had been found on January 3, 2012 and on January 24, 2012.
A service agreement included documentation that the facility has installed a "Night Watch Bed Bug Monitor" to address bed bug issues as they arise. Other preventative measures were documented during the physical plant inspection, such as taping off assembly holes in bunk beds, taping off foundation board edges, the installation of bait boxes, pest control inspections and pest control treatment services.

Plan of Correction The facility has continued to address client concerns regarding the appearance of any possible pests.
The policy & procedure manual has been updated, as shown to inspector at last visit, to reflect that every client who enters the facility will have their belongings placed into a high-heat dryer before being taken into the facility - as part of intake.
In addition, the facility has an ongoing contract with a pest control service, with the provider on-call in the event of any client concerns. This will be overseen by both the facility's clinical director & HR Director on a daily basis to prevent recurrence. |

(a) The Department may revoke or refuse to issue a license for any of the following reasons:
(1) Failure to comply with a directive issued by the Department.

Plan of Correction The facility has implemented a process of updating the DOH whenever a client complaint is received. Most recently, an incident report was submitted to the state on 2/16 detailing client concerns, treatment options taken, and the findings of no infestation by the on-call pest control expert. All reports will be filed within 24 hours of resolution.
To prevent recurrence, this will be handled going forward by the HR Director. |
