Observations
Based on observation during the physical plant inspection, the facility failed to provide an unobstructed exit from a counselor's office and the kitchen area.
The findings include:
A physical plant inspection was conducted on January 21, 2015. A padlock was placed on the outside of the counselor's office door as well as the door to the kitchen area. Therefore, these exits were obstructed.
These finding were reviewed with facility staff during the licensing process.

Plan of Correction
The clinical director physically removed the padlocks from the counselor's offices as well as the door to the kitchen door. The clinical director will inspect both facilities each month to ensure compliance to this regulation. |
