Observations Based on a review of page 4 of the "Staffing Requirements Facility Summary Reports" (SRFSR) and a telephone conversation with the Facility Director conducted on March 1, 2017, the facillity failed to ensure that all staff obtained at least four hours of tuberculosis, sexually transmitted diseases and other health related topics within the first year of employment. Employee #4 was hired in June 2015 as a Drug and Alcohol Treatment Specialist. The completion of four hours of tuberculosis, sexually transmitted diseases and other health related topics was due to be completed by June 2016. There was no documentation that this training was completed at the time of the inspection.

Plan of Correction Staff #4 has been informed on March 2, 2017 of the need for her to obtain at lease 4 hours of tuberculosis, sexually transmitted diseases and other health related topics by September 1, 2017. The President & CEO will be responsible for making certain that employee #4 has meet this requirement.
All required trainings will be monitored on a monthly basis by the Quality Assurance Committee and the Clinical Supervisor will be responsible for ensuring that all staff have completed the required trainings within the first year of employment. |
