INITIAL COMMENTS
 
This report is a result of a supervisory review of the written report of the findings of an on-site licensure inspection that was conducted on January 5, 2017 by staff from the Division of Drug and Alcohol Program Licensure. Based on this review, Pathway to Recovery Counseling and Educational Services was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this supervsiory review and a plan of correction is due on March 9, 2017.
 
Plan of Correction

704.11(c)(1)  LICENSURE Mandatory Communicable Disease Training

704.11. Staff development program. (c) General training requirements. (1) Staff persons and volunteers shall receive a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum. Counselors and counselor assistants shall complete the training within the first year of employment. All other staff shall complete the training within the first 2 years of employment.
Observations
Based on a review of page 4 of the "Staffing Requirements Facility Summary Reports" (SRFSR) and a telephone conversation with the Facility Director conducted on March 1, 2017, the facillity failed to ensure that all staff obtained at least four hours of tuberculosis, sexually transmitted diseases and other health related topics within the first year of employment. Employee #4 was hired in June 2015 as a Drug and Alcohol Treatment Specialist. The completion of four hours of tuberculosis, sexually transmitted diseases and other health related topics was due to be completed by June 2016. There was no documentation that this training was completed at the time of the inspection.
 
Plan of Correction
Staff #4 has been informed on March 2, 2017 of the need for her to obtain at lease 4 hours of tuberculosis, sexually transmitted diseases and other health related topics by September 1, 2017. The President & CEO will be responsible for making certain that employee #4 has meet this requirement.



All required trainings will be monitored on a monthly basis by the Quality Assurance Committee and the Clinical Supervisor will be responsible for ensuring that all staff have completed the required trainings within the first year of employment.
