This report is a result of an on-site licensure renewal inspection conducted on January 12, 2007 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on February 16, 2007.

(b) Intake procedures shall include documentation of:

Observations
Psychosocial evaluations in twelve of twelve client records failed to include an evaluation of the clients coping mechanisms, negative factors, and attitude toward treatment.

Plan of Correction
Executive Director will instruct Clinical Supervisor to hold a clinical meeting with all counselors and counselor assistant to advise that coping mechanisms, negative factors and attitude toward treatment must be included in all evaluations. The Executive director will have the Clinical Supervisor monitor all evaluations on an ongoing basis to ensure that this is done. |

(d) Provisions shall be made, through written agreement with a licensed hospital or physician, for 24-hour emergency psychiatric and medical coverage.

The governing body failed to provide documentation through written agreements for 24-hour emergency psychiatric and medical coverage.

The Executive Director will contact a local hospital and medical doctor and provisions shall be made through written agreemtns for 24-hour emergency psychiatric and medical coverage. |
