This report is a result of an on-site licensure renewal inspection conducted on March 9, 2022, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Casey Recovery LLC was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection: |

705.6 (4) LICENSURE Bathrooms.
705.6. Bathrooms.
The residential facility shall:
(4) Provide privacy in toilets by doors, and in showers and bathtubs by partitions, doors or curtains. There shall be slip-resistant surfaces in all bathtubs and showers.

Observations Based on a physical plant inspection, the facility failed to have slip-resistant surfaces in all bathtubs and showers. There were not slip-resistant surfaces in any of the showers located within the facility.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction The facility maintenance staff installed non-slip tape and tiles in all facility tubs and shower floors on 3/30/22. The Project Director inspected to ensure compliance on 4/1/22.
During regular, weekly facility inspections the Project Director or designated representatives shall ensure that all DDAP safety requirements are in compliance including areas of facility bathrooms and showers/tubs.
If any safety discrepancies are noted, the Project Director will instruct the facility maintenance staff to immediately fox or repair those issues. |
