Observations
Based on a physical plant inspection conducted during a complaint investigation on July 13, 2023, the facility failed to maintain each bathroom in a functional, clean and sanitary manner at all times.
Bathroom A, shower 1 was observed to have broken tile on the shower floor.
This was discussed with facility staff during the investigation.

Plan of Correction
A work order was completed for the repair of shower tile in bathroom A shower 1 on 7/14/23. Review of the physical plant to identify unresolved maintenance shall be completed at least quarterly or as needed by Facility Director. Ongoing responsibility of Facility Director. |
