INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on December 15, 2016, by staff from the Department of Drug and Alcohol Programs, Program Licensure Division. Based on the findings of the on-site inspection, B.I. Incorporated was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:
 
Plan of Correction

709.91(b)(6)  LICENSURE Intake and admission

709.91. Intake and admission. (b) Intake procedures shall include documentation of: (6) Psychosocial evaluation.
Observations
Based on a review of client records on December 15, 2016, the facility failed to document a psychosocial evaluation with a date in client records, #1, 2, 8 and 9. Client #1 was admitted on August 3, 2016. The psychosocial evaluation was not dated. Client #2 was admitted on August 29, 2016. The psychosocial evaluation was not dated. Client #8 was admitted on August 11, 2016. The psychosocial evaluation was not dated. Client #9 was admitted on June 22, 2016. The psychosocial evaluation was not dated. The findings were reviewed with the facility staff during the licensing inspection.
 
Plan of Correction
The Facility Director has trained Counselors on completing Psycho-social evaluations that is written and dated within 30 days of Intake. The Facility Director will review files monthly to ensure compliance. Corrections completed by 1/10/17.
