Observations
Based on a review of client records on December 15, 2016, the facility failed to document a psychosocial evaluation with a date in client records, #1, 2, 8 and 9. Client #1 was admitted on August 3, 2016. The psychosocial evaluation was not dated. Client #2 was admitted on August 29, 2016. The psychosocial evaluation was not dated. Client #8 was admitted on August 11, 2016. The psychosocial evaluation was not dated. Client #9 was admitted on June 22, 2016. The psychosocial evaluation was not dated. The findings were reviewed with the facility staff during the licensing inspection.

Plan of Correction
The Facility Director has trained Counselors on completing Psycho-social evaluations that is written and dated within 30 days of Intake. The Facility Director will review files monthly to ensure compliance. Corrections completed by 1/10/17. |
