INITIAL COMMENTS
 
This report is a result of a complaint investigation conducted on July 2, 2012 through July 3, 2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against PA Care, LLC d/b/a Miners Medical were unsubstantiated.
