INITIAL COMMENTS
 
This report is a result of an on-site inspection to this newly licensed facility following the admittance of its first client. The inspection was conducted on June 3, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of this on-site inspection, Pa Care Llc D/b/a Miners Medical was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on July 3, 2010.
 
Plan of Correction

704.11(c)(2)  LICENSURE CPR CERTIFICATION

704.11. Staff development program. (c) General training requirements. (2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.
Observations
Based on a review of facility staff CPR/first aid certification and a staff interview, the facility failed to ensure that at least one person trained in CPR skills onsite during the project's hours of operation.



Findings:



Based on a review of staff schedules and staff CPR/first aid certification, the facility had only one person on staff who was certified to provide CPR and first aid. In a staff interview, it was acknowledged that having only one staff certified to to provide CPR and first aid was not enough to handle the coverage needed for all hours of operation.
 
Plan of Correction

705.28 (c) (4)  LICENSURE Fire safety.

705.28. Fire safety. (c) Fire extinguishers. The nonresidential facility shall: (4) Instruct staff in the use of the fire extinguisher upon staff employment. This instruction shall be documented by the facility.
Observations
Based on a review of personnel records, the facility failed to document the instruction of staff in the use of the fire extinguisher upon staff employment, in four of four personnel records.



The findings include:



Four personnel records were reviewed on June 3, 2010; all were required to provide documentation of instruction to use the fire extinguisher upon staff employment. Personnel records 1, 2, 3 and 4 contained documentation of fire drill training, however there was no date on the documentation to verify that the instruction had provided upon staff employment.

709.26(d)(4)  LICENSURE Personnel Management

709.26. Personnel management. (d) The personnel records shall include, but not be limited to: (4) Salary information.
Observations
Based on a review of personnel records and a staff interview, the facility failed to include salary information in four of four personnel records.



The findings include:



Four personnel records were reviewed on June 3, 2010; all records were required to include salary information. None of the personnel records included salary information.

709.44(a)(1)(i)  LICENSURE Medical History

709.44. Client records. (a) The project shall maintain a client record on an individual which shall include, but not be limited to: (1) Histories, which include the following: (i) Medical history.
Observations
Based on a review of four client records, the facility failed to document a medical history to include the client's family's medical history in three of four client records.



The findings include:



Four client records were reviewed on June 3, 2010; all were required to contain documentation of a medical history. Client records 1, 2 and 4 failed to document a medical history that included the family medical history.
