Observations
Based on a review of three consent to release forms that was incorrectly faxed to DDAP on May 21, 2021, the facility failed to ensure the clients confidentiality due to the fact that the forms included the clients full name, birth date and full social security number.
These findings were discussed with facility staff during a phone conversation.

Plan of Correction
Miners Medical failed to comply with regulation 709.28 Confidentiality (C). The clinical supervisors discussed individually with each employee the importance of faxing confidential information to the correct facilities. The fax number to the Department of Drug and Alcohol was removed from the fax machine immediately. |
