This report is a result of an on-site relocation physical plant licensure inspection conducted on November 5, 2024 by staff from the Bureau of Program Licensure. Based on the findings of the on-site relocation inspection, Cleanslate Medical Group of Pennsylvania, LLC - Bethlehem was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Deficiencies were identified during this inspection and a plan of correction is required.

(b) The licensee, using Department forms, shall notify the Department within 90 days of the occurrence of any of the following conditions:

Observations
The facility failed to notify the Department within 90 days of a change in location as evidenced by the following:
Per a conversation with facility staff on November 4, 2024 it was reported that the facility was relocated on July 26, 2024. However, the Department was not informed of the relocation until the facility submitted their relocation documentation which was dated October 31, 2024.

Plan of Correction
CleanSlate Medical will ensure to inform DDAP of any relocation of our offices before the 90 days required time frame. CleanSlate's Regulatory Compliance team will be responsible for the notification to DDAP in the future. |
