INITIAL COMMENTS
 
This report is a result of a methadone intervention conducted on October 25- November 1, 2019, by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention & Treatment. Based on the findings of the on-site inspection, New Directions Treatment Services, was found to be not in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this investigation
 
Plan of Correction

715.22(b)  LICENSURE Patient grievance procedures

(b) The procedure shall permit aggrieved patients a full and fair opportunity to be heard, to question and confront persons and evidence used against them and to have a fair review of their grievances by the narcotic treatment program director. If the grievance is filed against the narcotic treatment program director, the review of the case shall be conducted by either a multi-representative group of the narcotic treatment program or a subcommittee of the governing body instituted for the express purposes of grievance adjudication.
Observations
The facility failed to adhere to their policy regarding client grievances.On 10/17/19 a client requested a meeting with her counselor to discuss filing a grievance. An email correspondence submitted to DDAP staff confirms that at least two facility staff members were aware of this client ' s desire to initiate the grievance process, however the client was not permitted to file the grievance at this time but was given an appointment on 10/21/19.
 
Plan of Correction
On November 12, 2019, the Director of Operations, Clinical Supervisor, and Program Director facilitated a staff meeting with the counselors to review and address the clinic's grievance procedure. As soon as a client expresses a problem about an issue that they feel is impacting their treatment, the client's counselor will assist the client to problem solve the matter and identify solutions. If the counselor needs assistance in resolving the situation with the client, a supervisor will be notified immediately (either Clinical Supervisor or Program Director) to offer guidance, solutions, and support. If a client expresses an interest in filing a grievance, and mentions the word "grievance," a form will be provided to the client, immediately. Whoever is available immediately, either the counselor, Clinical Supervisor, Program Director, or any staff member who is not the subject of the grievance, will assist the client to complete the form. To ensure ongoing compliance, the Program Director and Clinical Supervisor will, during weekly group supervision, ask counselors if any clients on their caseloads have any issues/concerns to address and resolve.
