INITIAL COMMENTS
 
This report is a result of an on-site complaint investigation conducted on June 9, 2026, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation,Clearbrook Treatment Centers, LLC d/b/a Huntington Creek Recovery Center , was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
 
Plan of Correction

709.32 (c) (1) (i) - (ii)  LICENSURE Medication control

§ 709.32. Medication control. (c) The project shall have and implement a written policy and procedures regarding all medications used by clients which shall include, but not be limited to: (1) Administration of medication, including the documentation of the administration of medication: (i) By individuals permitted to administer by Pennsylvania law. (ii) When self administered by the client.
Observations
Based on the review of medication administration records (MAR,) the facility failed to document the administration, or the reason a medication was not administered.

Client #1 was admitted on March 2, 2026, and discharged on April 23, 2026. On March 5, 2026, Client #1 was prescribed Effexor 37.5mg to be administered once a day. On April 19, 2026, there is no documentation in the MAR to verify this medication was administered.

Additionally, on March 19, 2026, Sublocade 300mt/1.5. was ordered for Client #1. There is no documentation in the MAR to verify that this medication was administered.

These findings were reviewed with facility staff during the complaint investigation.
 
Plan of Correction
Each nurse administering medications on their shift will verify all routine medications are documented correctly on the MAR. As a means to validate completion, the nurse will also ensure the routine MAR for completeness of the previous med pass administration. The Director of Nursing provided education and instruction to the nursing staff on 06/12/26.

The Director of Nursing will review ten MAR per month for three months to verify completeness.
