INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on October 21, 2025, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Nanticoke Medical Center LLC was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:
 
Plan of Correction

705.2 (2)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.
Observations
Based on a physical plant inspection, the facility failed to keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of clients, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.



It was observed by DDAP staff that the facility's front walkway is in disrepair with pieces of concrete missing in spots presenting a slip, trip and fall hazard.



These findings were reviewed with facility staff during the licensing process.



This is a repeat citation from November 20, 2024 licensing inspection.
 
Plan of Correction
There was chipped paint on the column inside the building , the paint was stripped and repainted. The maintenance person will inspect thru out the building for paint inspection and if any defect to repair immediately.



The sidewalk has chipped cement this was previously fixed and damaged with snow removal plow. This has been repaired. The maintenance person will inspect every month for any damage to concrete, and in winter only use snow shovel to remove snow instead of plow. In first week

Of 2026 we will completely replace the pathway when it will be warm enough to lay new concrete sidewalk. After the replacement of the side walk maintenance person will inspect pathways every month for cleanliness and to inspect for any damage.
