This report is a result of an on-site licensure renewal inspection conducted on May 5, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection BN2B6701 Stephens Place Inc, was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, deficiencies were identified during this inspection and a plan of correction is required.

§ 717.22(b)(3)(vi) LICENSURE Beginning of Residency
§ 717.22. Beginning of residency.
(b) The licensee shall complete the following documentation, which must be signed by the resident, within 24 hours of arrival:
(3) Basic personal data including:
(vi) Medical contact information.

Observations
Based on review of the resident records, the recovery house failed to collect medical provider information in 6 out of 6 records. These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
Stephen's Place staff have made necessary corrections and adjustments on our application, indicating medical provider information. This will been filled out during orientation of new residents. We have added those line questions to our form and will collect that information going forward.
The Program Director will ensure that the corrective actions have been implemented.
The corrective actions have been completed on 5/8/2025, and a copy of the form was emailed to the DDAP Recovery House inspector. |
