INITIAL COMMENTS
 
This report is a result of an off-site complaint investigation conducted October 6-8, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the complaint investigation, Wilkes-Barre Treatment LLC DBA Clearbrook Treatment Centers was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
