Observations The facility failed to ensure that 6 support staff employees received an annual training plan for the current training year, based on a review of the facility's Staffing Requirement Facility Summary Report and a discussion with the project director.Employee #8 was hired as an administrative assistant on August 8, 2015.Employee #9 was hired as an administrative assistant on March 31, 2005. Employee #10 was hired as a security officer on December 12, 2012. Employee #11 was hired as a security officer on February 21, 2008. Employee #12 was hired as an administrative assistant on September 3, 2002.Employee #13 was hired as an administrative assistant on January 31, 1989.The findings were discussed with facility staff during the licensing process.

Plan of Correction On 11/2/2018, the Program Director informed the Director of Operations that individual training plans were not completed by support staff employees. On 11/2/2018, employees #8, #9, #10, #11, #12, and #13, completed the Individual Training Plans, identifying goals and learning objectives that they would like to work on. The Director of Operations will make sure that all of the support staff employees complete their individual training plans every June, identifying goals and learning objectives that they would like to focus on for the upcoming year. |
