INITIAL COMMENTS
 
This report is a result of an onsite complaint investigation conducted on May 31, 2024, by staff from the Bureau of Program Licensure. Based on the findings of the complaint investigation Nehemiah Recovery House, was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the recovery house. Therefore, deficiencies were identified during this investigation and a plan of correction is required.
 
Plan of Correction

§ 717.29(f)(2)  LICENSURE Physical plant standards - Kitchens

§ 717.29. Physical plant standards. (f) Kitchens. The licensee shall: (2) Ensure that storage areas for foods are free of food particles, dust and dirt.
Observations
Based on physical plant inspection the recovery house failed to keep storage areas for food free of food particles, dust and dirt.
 
Plan of Correction
The House Manager is responsible for ensuring that all storage areas are free of food particles, dust and dirt. The House Manager executed a thorough cleaning of the storage areas to ensure that they are free of food particles, dust and dirt on 6/1/24. The House Manager will inspect the storage areas weekly to ensure that the storage areas remain free of food particles, dust and dirt.

§ 717.30(d)(3)  LICENSURE Safety and emergency procedures -Extinguisher

§ 717.30. Safety and emergency procedures. (d) Fire extinguishers. The licensee shall: (3) Ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company. The date of the inspection must be indicated on the extinguisher or inspection tag; the licensee shall replace or repair a fire extinguisher found to be inoperable within 48 hours.
Observations
Based on physical plant inspection the recovery house failed to ensure that fire extinguishers are inspected annually.
 
Plan of Correction
The House Manager is responsible for ensuring that all fire extinguishers are up to date. The fire extinguishers were inspected and tagged for 2024 on 6/12/24. The House Manager will ensure that all fire extinguishers are inspected and tagged 1 month prior to expiration date and annual inspection.
