Observations
Based on a review of resident records, the recovery house failed to provide a written notification to the resident stating the decision to end residency. These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
Because the resident is no longer a resident at Stephen's Place, we cannot go back and correct that particular situation. However, moving forward, the Program Director will place a copy of the Notification to End Residency form in each of the resident's files to ensure that this deficiency will not occur again. The Program Director will be responsible for ensuring the corrective action is implemented, and the corrective action will be implemented immediately (4/15/26). We have emailed a copy of the Notification to end residency form to our DDAP inspector for approval. |
