INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on June 13, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection MM006701 Empower Works was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, deficiencies were identified during this inspection and a plan of correction is required.
 
Plan of Correction

§ 717.29(c)(2)  LICENSURE Physical plant standards - Living room-Lounge

§ 717.29. Physical plant standards. (c) Living rooms and lounges. The licensee shall: (2) Maintain furnishings in a state of good repair.
Observations
Based on the physical plant inspection the recovery house failed to ensure furnishings in a state of good repair.



These findings were reviewed with the recovery house staff during the licensing process.
 
Plan of Correction
The House Manager will ensure that the dining room tablet is fixed and safe. The glass in the table will be replaced or another material will replace the glass. This will be completed by June 30, 2025. The resident lead will do a biweekly check to ensure that all furnishing is in good repair. If there are things that need repaired, they will report it to house manager immediately. If something needs repaired, a solution will be sought immediately.

§ 717.30(d)(3)  LICENSURE Safety and emergency procedures -Extinguisher

§ 717.30. Safety and emergency procedures. (d) Fire extinguishers. The licensee shall: (3) Ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company. The date of the inspection must be indicated on the extinguisher or inspection tag; the licensee shall replace or repair a fire extinguisher found to be inoperable within 48 hours.
Observations
Based on the physical plant inspection the recovery house failed to ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company.

Plan of Correction
COMPLETED: All fire extinguishers were inspected on 6/17/25.



The House Manager will ensure that all fire extinguishers are inspected annually.
