INITIAL COMMENTS
 
This report is a result of an onsite complaint investigation conducted on December 5, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the onsite complaint investigation, Clearbrook Treatment Centers, LLC d/b/a Huntington Creek Recovery Center, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
 
Plan of Correction

709.24 (a) (3)  LICENSURE Treatment/rehabilitation management.

§ 709.24. Treatment/rehabilitation management. (a) The governing body shall adopt a written plan for the coordination of client treatment and rehabilitation services which includes, but is not limited to: (3) Written procedures for the management of treatment/rehabilitation services for clients.
Observations
Based on direct observation by DDAP staff, the facility failed to adhere to its written procedures for the management of treatment/rehabilitation services for clients.The facility ' s smoking policy "QC042-Smoking on Facility Property" notes "any form of smoking (tobacco, electronic devices, etc.) is prohibited inside of any building on thefacility ' s property."Upon entry into the facility DDAP staff observed a client vaping indoors in the facility lobby. This finding was reviewed with facility staff during the complaint investigation process.
 
Plan of Correction
Upon a client's admission, patient support staff will continue to inform/educate them on the facility's smoking policy at orientation. Department leadership will review the smoking policy with their teams in a group supervision and instruct staff to immediately address any client that is violating the policy by 01/31/26. Patient support staff and clinical staff will remind clients of the smoking policy every day during a morning meeting. Clients who violate the policy will be addressed individually by their counselor and may be placed on a behavioral plan if assessed as necessary for that client.
