Observations Based on a review of patient chart documentation, and the Patient Rights, it appears the Patient Rights were not adhered to. The Patient Rights dictate that patients are to be treated in a manner that promotes dignity and self-respect. To receive services free from retaliation, humiliation, and/or neglect. To receive services in accordance with standards of professional practice, and services appropriate to individual needs.
Patient # 1 was admitted 1/24/23 and discharged 8/1/24. Documentation shows the patient saw the facility physician on June 5, 2024, and the physician did not notify the pharmacy to reorder a prescription. The patient ran out of medication on June 5, 2024, and it was not reordered until June 7, 2024.

Plan of Correction Clinic Director met with medical providers on 8/12/24 and 8/13/24 to review patient rights to receive services. To ensure continuity of care and ongoing maintenance, medical providers will ensure their is no gap in medication regimen/presecription. The frequency and dosage for the prescription can and will be at the discretion of the prescribing physician. In such situations pt. could be offered daily dosing and adjust Rx accordingly. Clinic Director will monitor for compliance in this area. |
