Observations
Based on the physical plant inspection, the facility failed to ensure that all heaters were permanently mounted or installed as a space heater was discovered in the treatment team office on the third floor.This finding was reviewed with facility staff during the licensing inspection.

Plan of Correction
1.Clinical Supervisors will conduct search of each residential program for heaters. If a heater(s) is found the item will be removed from facility.

2. Clinical Supervisors will inform employees that free standing heaters are not permit in the facility, during next staff meeting.

3. Clinical Supervisors will survey their assigned program to determine, if there a need for heat in a certain area

4.Operation and building maintenance will troubleshoot the possible cause of forced hot air not fully circulating in the building i.e. . Clogged air filters, thermostat problems, issues with the blower motor and HVAC system, etc. The purpose of troubleshooting HVAC system is to address cold spots in the building
