This report is a result of an on-site complaint investigation conducted on July 29, 2015, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Eagleville Hospital were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
