INITIAL COMMENTS
 
This report is a result of an onsite complaint investigation conducted on June 26, 2018 by staff from the Bureau of Quality Assurance for Prevention & Treatment. Based on the findings of the telephone investigation, Habit OPCO, Inc-Pottstown was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
