This report is a result of a complaint investigation conducted by telephone on November 13, 2009 and November 16 and 17, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Habit OPCO were substantiated, however, the Project Administrators promptly and appropriately took action in accordance with the Project's policy and procedure. No further action is required by the Project.
