This report is a result of an on-site follow-up investigation conducted on August 8, 2016 by staff from the Division of Drug and Alcohol Program Licensure in response to Incident Reports that was submitted by the facility. Based on the findings of the investigation, Eagleville responded appropriately to the incident. Therefore, the facility was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
