INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on September 16-17, 2026, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Rosewood Recovery LLC was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

704.11(c)(1)  LICENSURE Mandatory Communicable Disease Training

704.11. Staff development program. (c) General training requirements. (1) Staff persons and volunteers shall receive a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum. Counselors and counselor assistants shall complete the training within the first year of employment. All other staff shall complete the training within the first 2 years of employment.
Observations
Based on a review of personnel records and the facility's Staffing Requirement Facility Summary Report (SRFSR) form, the facility failed to ensure that employees received at least 4 hours of TB/STD and other health related topics using a Department approved curriculum within the regulatory timeframe.Employee #4 was hired as a counselor on April 9, 2025, and was still in the position as of the date of the inspection. Employee #4 was due to complete the communicable disease trainings no later than April 9, 2026. There was no documentation in the personnel file for the completion of the TB/STD training using a Department approved curriculum as of the date of the inspection. The findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Project Director updated Rosewood Recovery's standard training plan guide on September 18, 2026, to reflect the requirement for four hours of TB/STD training using a Department-approved curriculum.

On September 21, 2026, the Project Director notified staff who had not yet completed the approved training of the appropriate training they should register for and advised them of the required timeframe for completion. The identified staff was asked to register for the TB/STD training by September 30, 2026 and will continue to check for open classes that are local or virtual.

Moving forward, the Project Director will review staff training requirements and completion status at least monthly during supervision to ensure ongoing compliance with Department training requirements.

Observations
Based on a review of the Staffing Requirements Facility Summary Report and personnel records, the facility failed to document the completion of 25 clock hours of annual training required for counselors. Employee #3 was hired as a counselor on December 23, 2024 and was still in the position as of the date of the inspection. Employee #3 ' s record for the reviewed training year, January 1 - December 31, 2025, documented only 22 hours and 25 minutes out of the required 25 hours of annual training. This finding was reviewed with facility staff during the licensing process.

Plan of Correction
The Project Director miscalculated the total training hours completed by Employee #3 for the 2025 training year. Moving forward, the Project Director will regularly review and reconcile staff training certificates to ensure that each employee meets the required 25 hours of annual training.
