INITIAL COMMENTS
 
This report is a result of an on-site complaint investigation conducted on June 12, 2024, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Malvern Institute for Psychiatric and Alcoholic Studies was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
 
Plan of Correction

705.2 (2)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.
Observations
During a complaint investigation conducted on June 12, 20224, it was observed that the facility was not kept in good repair at all times for the safety and well-being of residents, employees and visitors.The vinyl plank flooring of the Executive Conference Room was observed to have some strips of planking that were missing, some sections cut out on the vinyl, and one vinyl planking strip that was torn in two places.

Plan of Correction:



Identification of Failure: We are currently in the process of replacing the flooring throughout the facility and were focusing on patient specific areas first.





Action: The Director of Plant Operations scheduled the replacement of the flooring in the conference room. The conference room flooring was replaced on 6/18/2024.





Monitoring:

Monitoring will be completed monthly by the Director of PI and the Director of Plant Operations during their monthly walkthrough safety and environmental rounds to establish compliance with keeping the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. Any deficiencies identified will be discussed in the monthly safety meeting and a plan for resolution will be implemented by the committee and assigned to the responsible party. Compliance will be established when the project is completed and approved by the safety committee.
