INITIAL COMMENTS

This report is a result of a complaint investigation conducted on May 13, 2022 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the complaint investigation, the allegations against Habit Opco, Inc. were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
