This report is a result of a renewal on-site licensure inspection conducted on September 22, 2026, by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, L2LP6701 Step By Step Houses Llc was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing. Therefore, deficiencies were identified during this inspection and a plan of correction is required.

Observations
Based two of four resident records reviewed. The facility failed to have documentation signed by the residents within twenty-four hours of arrival. Resident # 1 was admitted on June 26, 2026, and was still active at the time of the inspection. Documentation for beginning residency was not signed by the resident until June 30, 2026.Resident # 2 was admitted on December 12, 2025, and was still active at the time of the inspection. Documentation for beginning residency was not signed by the resident until December 16, 2026, and February 11, 2026.These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
The House Manager will go over and train the staff the exact way that is required the day residents arrive and to have paperwork filled out and signed within 24 hours of their arrival. The House Manager will review the records on a consistent basis to make sure everything is completed |
