This report is a result of an on-site recovery house complaint investigation conducted on September 9, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Blue Mountain Escape 7WNK6701 was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(3) Keep the drug and alcohol recovery house free of rodent and insect infestation.

Observations
Based on an inspection of the physical plant, the recovery house failed to keep the recovery house free of rodent infestation.

Plan of Correction
The recovery house failed to keep the house free from rodents.
On 09/17/2026 the Lead cleaned the droppings from the mice.
On 09/18/2026 Ehrlich Pest Control conducted the initial treatment placing mouse traps throughout the property. Additionally, they covered the holes on the foundation where mice can enter. The Director agreed to a service plan with Ehrlich to remain pest free. The Director will continue to monitor with the Lead weekly. |
