Observations Based on the physical plant inspection conducted on June 5, 2025, the facility failed to ensure privacy so that counseling sessions cannot be seen or heard outside the counseling room.
The large group room in the basement of the facility, did not have window covering on multiple window; therefore, all sessions could be seen from individuals outside in the facility ' s yard area.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction As of 6/17/2025, Saint John Vianney Center installed Window Obscuring Film on all windows of the Lower-Level Group Room to eliminate the ability to identify persons during group sessions. This method provides a lasting solution to ensure all future counseling sessions are private.
To ensure on-going privacy of group participants, the condition of the window treatment will be evaluated quarterly as part of the facility's Environmental Risk Assessment monitoring. Any window treatment repairs needed will be completed immediately by the Facilities Department.
