This report is a result of a complaint investigation conducted by telephone on August 30, 2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the telephone complaint investigation, the allegations against Gaudenzia DRC were unsubstantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
