This report is a result of an initial on-site physical plant inspection regarding a facility relocation. The inspection was conducted on October 18, 2016 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the physical plant inspection, Foundations Medical Services LLC was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility, specifically 28 Pa. Code Subsection 705.1. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
Since this was a physical plant inspection following a facility relocation, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
