INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on November 14, 2024 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, My Sister's Place Thomas Jefferson University was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:
 
Plan of Correction

711.53(c)(2)(vi)  LICENSURE Consent's Expiration Date

711.53. Client records. (c) Confidentiality. (2) The project shall obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record. The consent shall be in writing and include, but not be limited to: (vi) The expiration date of the consent.
Observations
Based on a review of client records, the facility failed to document the expiration date of the consent on consent to release information forms in two of seven client records reviewed.



Client # 3 was admitted on June 25, 2024 and was active at the time of inspection. A release of information form to a government agency was signed by the client on July 30, 2024, but the release form did not include the expiration date of the consent form.



Client # 6 was admitted on March 1, 2024 and was discharged August 12, 2024. A release of information form to an outside treatment provider was signed by the client on June 25, 2024, but the release form did not include the expiration date of the consent form.



These findings were reviewed with facility staff during the licensing process.
 
Plan of Correction
Residential Clinical Director and Clinical Supervisor will review My Sister's Place policies and procedures as it relates to obtaining and completing consents to release information with clients. A complete review of how to complete and ensure expiration dates are on each consent. Clinical Director and Clinical Supervisor will review the importance of complete and accurate consents. For all new consents obtained, completing staff member will ensure that a expiration date is on all consents before signatures are obtained.



For active client record, the Patient Navigator will complete new consents with client and include appropriate expiration date.





Clinical Supervisor will review all records regularly to ensure that all consents are completed correctly.



This Plan of Correction will be implemented by 12/06/2024
