INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted May 6, 2025 through May 7, 2025, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Malvern Institute for Psychiatric and Alcoholic Studies, Inc., was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:
 
Plan of Correction

705.6 (2)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (2) Provide a sink, a wall mirror, an operable soap dispenser, and either individual paper towels or a mechanical dryer in each bathroom.
Observations
Based on a physical plant inspection, the facility failed to provide a sink, a wall mirror, an operable soap dispenser, and either individual paper towels or a mechanical dryer in each bathroom.



At the time of the inspection, conducted on May 7, 2025 at approximately 11:30am, there were missing or empty paper towel dispensers and no mechanical dryer in the bathrooms located in rooms 202, 209, and 210.



These findings were reviewed with facility staff during the licensing process.
 
Plan of Correction
Identification of Failure: The paper towel dispenser was missing from the bathroom wall and two additional bathrooms had empty dispensers.



Action: The missing paper towel dispenser was immediately replaced and the paper towels were refilled.



Monitoring: The lead housekeeper will monitor the facility daily to assure all dispensers are available and are refilled each morning. The facility will be monitored daily with a goal of 100% for 3 months.
