This report is a result of an on-site licensure renewal inspection conducted on June 7, 2007 through June 8, 2007 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on July 3, 2007.

Observations
Follow-up information was not documented in two of four records reviewed, #6 and 7. Follow up information was not documented within the time frame identified by the facility in two of four records reviewed, #5 and 8.
