INITIAL COMMENTS

This report is a result of an on-site recovery house complaint investigation conducted on September 18, 2026, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, TNH W8WJ6701 was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

Plan of Correction

§ 717.29(b)(1)  LICENSURE Physical plant standards - Bldg-Exterior-Grnd

§ 717.29. Physical plant standards. (b) Building interior, exterior and grounds. The licensee shall: (1) Keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.
Observations
Based on an inspection of the physical plant, it was found the recovery house failed to keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.

The following items were found and reviewed with Recovery House staff:

A flood in the basement resulting in standing water and a black, mold-like substance observed on the ceiling of the second floor bathroom and ventilation fan.

Plan of Correction
TNH initiated corrective action to address both the observable substance and the underlying moisture concerns.

The second-floor bathroom ceiling and ventilation area were inspected, cleaned, and appropriately treated/remediated. The bathroom ventilation system was inspected to ensure that it is functioning appropriately.

The basement water intrusion was also evaluated to identify and correct the source of the moisture. Affected areas were cleaned and dried, and necessary maintenance/repair was initiated to prevent continued water intrusion.

Corrective Action Completed By: TNH Maintenance Technician

Verification of Completion:

District/Program Management will inspect both areas following completion of corrective work to verify that the areas are clean, dry, sanitary, and in good repair. Any recurrence of moisture or mold-like material will result in additional maintenance evaluation and corrective action.

Prevention of Recurrence

Monthly Residential Quality Control inspections will specifically examine:

Bathrooms and ventilation systems;

Ceilings and walls for discoloration or evidence of moisture;

Basements for standing water, leaks, or water intrusion;

Areas surrounding plumbing fixtures;

Evidence of mold or mildew;

Water-damaged materials; and

General sanitation and physical condition.

Resident Leads will be instructed that water intrusion, active leaks, or suspected mold/mildew conditions are not routine housekeeping issues and must be reported immediately to District/Program Management.
