This report is a result of an on-site complaint investigation conducted on January 14, 2026, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Clearbrook Treatment Centers, LLC d/b/a Huntington Creek Recovery Center, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. |

705.7. Food service.
(b) A residential facility may operate a central food preparation area to provide food services to multiple facilities or locations. A residential facility that operates an onsite food preparation area or a central food preparation area shall:
(5) Keep cold food at or below 40F, hot food at or above 140F, and frozen food at or below 0F.

Observations Based on a physical plant inspection, the facility failed to keep hot food at or above 140 F.
DDAP staff observed the facility's lunch. The facility's cheese steak meat was measured at 120 F at the time of serving.
This finding was reviewed with facility staff during the licensing inspection process.

Plan of Correction The Manager of Dietary Services has provided additional education and instruction to the dietary staff. Food in the steam table will be checked for proper temperature before going out to be served. The food will be monitored during service multiple times and will be reheated if the temperature is not at least 140F. The Manager of Dietary Services will conduct random spot checks to ensure that these steps are occurring. |

(6) Store all food items off the floor.

Observations Based on a physical plant inspection, the facility failed to store all food items off the floor. DDAP staff observed cooking oil being stored on the floor of the facility pantry.
This finding was reviewed with facility staff during the licensing inspection process.

Plan of Correction The oil was immediately removed from the floor and stored on a shelf. The Manager of Dietary Services provided education and instruction to the staff on the proper storage of food items and not storing any food items on the floor. The Manager of Dietary Services will conduct random spot checks to ensure that staff are adhering to the proper storage of food. |
