This report is a result of an on-site complaint investigation conducted on June 6, 2019, by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention & Treatment. Based on the findings of the on-site inspection, Eagleville Hospital, was found to be not in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this investigation. |

Observations On June 6, 2019 DDAP staff conducted an on-site investigation. The investigation included staff and client interviews, and a physical plant inspection.
Based on interviews with staff and residents it was confirmed that there was a lack of air conditioning in part of the identified unit of the facility from April 25-30, 2019.

Plan of Correction On May 28, 2019 in response to a call from DDAP regarding a patient/ family complaint of high temperatures on the unit, temperature measurements were taken throughout the building in question (D'Arclay) by building maintenance.
While the temperatures were within the range of 65 to 90 degrees, as specified by the regulations, an air conditioning malfunction was contributing to the temperature range being 72.3 to 77.3 with a target rate of 72 to 74 degrees.
The assessment of the HVAC system identified a malfunctioning evaporator coils.
Our action plan to maintain our target temperature within the affected areas includes: (1) Immediate placement of 8 portable air conditioning units for use from May 29, 2019 until HVAC unit are completely repaired. (2) Parts to be ordered and HVAC contractor obtained to repair equipment.
Upon receipt of newly manufactured evaporative coils for the current HVAC equipment, install coils to correct air conditioning problem in June, 2019. (3) Evidence of Compliance for temperature range using the temporary portable air conditioning units will be conducted using our temperature-control software program that provides alerts for high and low temperatures. If high temperatures occur corrective action will be documented in the maintenance request system through the end of June, 2019
