INITIAL COMMENTS
 
This report is the result of an on-site complaint investigation conducted on September 2, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site investigation Eagleville Hospital was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The allegation was substantiated and the following deficiency was identified during this inspection. A plan of correction is due on October 16, 2010.
 
Plan of Correction

705.9 (1)  LICENSURE General safety and emergency procedures.

705.9. General safety and emergency procedures. The residential facility shall: (1) Be free of rodent and insect infestation.
Observations
Based on a physical plant tour, staff interviews, facility maintenance records, and personnel e-mail correspondence, and the facility failed to ensure the grounds were free of rodent and insect infestation.



The findings include:





A physical plant tour was conducted on September 2, 2010. At approximately 2:20 PM three pin point-sized dots which were red in color and appeared to be consistent with blood stains after a bug bite were found on the pillow of a bed in bedroom number 104 in the Levy building. The maintenance director confirmed the stains on the pillows.

On September 2, 2010 interviews were held with the facility director and maintenance director. During the interviews it was confirmed that the facility had been exposed to bed bugs. The maintenance director explained the procedures that the facility was taking in an attempt to eliminate the bedbugs.

The facility did provide documentation verifying that a pest control service had been to the facility to treat for bed bugs on August 16, 2010 and a follow up service was conducted on August 25, 2010.
 
Plan of Correction
A pest control service was in the facility to treat bed bugs on the first floor of the Levy Building on August 16 and 25, 2010. This treatment eradicated the bed bugs in this area. There was no confirmation on September 2, 2010 or anytime thereafter of remaining or additional bed bugs on the first floor of the Levy Building.



Eagleville Hospital maintains a service contract for pest control and eradication. Pest control services are provided on a routine and as-needed basis. Policies and procedures are in place for the identification, communication and servicing of any problems identified with pests.



A policy and procedure specific to bed bugs will be developed. The policy will include procedures for identification, control and elimination of any bed bugs identified within the facility. This policy will be distributed to applicable staff. Monitoring of bed bugs within the facility will be completed through documentation in the Pest Control Service Log. The engineering supervisor will be responsible for implementing this plan.
