This report is a result of an onsite licensure renewal inspection. The inspection was conducted on December 20-21, 2016 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the onsite inspection, Foundations Medical Services, LLC was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
