This report is a result of an offsite intervention conducted by phone. The intervention was conducted on June 28, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the discussion with the Facility Director, the intervention was completed. No deficiencies were identified during this intervention and no plan of correction is required.
