INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on October 16, 2019 the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment. Based on the findings of the on-site inspection, Gateway Rehabilitation Center Inc. Moffett House was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:
 
Plan of Correction

704.11(c)(1)  LICENSURE Mandatory Communicable Disease Training

704.11. Staff development program. (c) General training requirements. (1) Staff persons and volunteers shall receive a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum. Counselors and counselor assistants shall complete the training within the first year of employment. All other staff shall complete the training within the first 2 years of employment.
Observations
Based on a review of personnel records, the facility failed to provide documentation of TB/STD training in one support staff record.



Employee #6 was hired as a counselor tech on January 23, 2017. TB/STD training was due to be completed by January 23, 2019. There was no documentation of TB/STD training in employee #6's record as of October 16, 2019.





These findings were reviewed with facility staff during the licensing process.
 
Plan of Correction
Employee #6 has registered for DDAP TB/STD training on 12/10/19. Staff will submit training certificate to GRC HR department and give copy to Moffet House manager to place in staff file.

To ensure staff at Gateway Rehab receives the training for Communicable Disease that is required within 365 days of hire for all employees, Gateway Rehab designate trainer(s) who will use the DDAP approved curriculum for TB/STD and HIV/Aids (Licensing Alert 6.1997). Gateway Rehab will offer this training one time every two months starting January 1, 2020 to ensure all staff have an opportunity for training.



Additionally, Gateway Rehab Executive Management has approved to withhold pay increases of staff who fail to comply with the mandatory training requirements.



Employees who have not complied with the mandatory training requirements are registered to complete the required trainings by 1/1/2020.



The applicable policy and procedure will be reviewed and revised to reflect these changes.
