The Drug and Alcohol Facility Information page displays a list of facilities that match the search criteria
entered on the Drug and Alcohol Facility Locator page. The search criteria entered are summarized
at the top of the Drug and Alcohol Facility Information page.
Basic demographic information about each facility is provided as well as links to each of the facilities
survey results and additional services.
Description of Data Displayed on Drug and Alcohol Facility Information Page
For each facility displayed on the page, the following facility specific information is also provided:

| Name/address/telephone number |
The name under which the facility does business (d.b.a. or fictitious name) appears in bold. If the facility's owner name is different, that name appears below the d.b.a. within parenthesis and in italics (owner's name). To the right of the name, RH appears within parenthesis for a Recovery House facility, while D/A appears within parenthesis for a Drug and Alcohol Treatment facility, when filtered to both facility types. |

Indicates whether the facility is for profit or not for profit. |

| Last inspection date |
This is the date of the last completed inspection (licensure renewal or complaint investigation) for which survey results can be found via this web site. The survey results are placed on the web site approximately 45 days following the exit date of the on-site licensure inspection or the conclusion of the complaint investigation. |

| Activity and date added - client capacity - licensure status |
Activity and client capacity - identifies the drug and alcohol
activities and date added for which the facility is licensed and the
maximum number of clients or beds. For nonresidential activities,
this is the maximum number of clients a facility is authorized to
treat at any one time for the identified activity. For residential
facilities, this is the maximum number of beds in which the
identified activity can occur at any one time.
Licensed activities include:
· Intake, evaluation and referral
· Inpatient non-hospital detoxification
· Inpatient non-hospital drug-free
· Inpatient non-hospital other chemotherapy
· Inpatient non-hospital transitional living facility
· Inpatient hospital detoxification
· Inpatient hospital drug-free
· Inpatient hospital other chemotherapy
· Psychiatric hospital
· Partial hospitalization drug-free
· Partial hospitalization other chemotherapy
· Outpatient detoxification
· Outpatient maintenance
· Outpatient drug-free
· Outpatient other chemotherapy
Licensure status - full licensure status
means that the requirements for licensure have been met. Provisional
licensure status means that the facility has substantially but not
completely complied with a sufficient number of the requirements for
full licensure. Provisional licensure is also issued to new
facilities following an initial on-site inspection.

Accessing Facility Survey Results
Clicking the link labeled “View Surveys” under the facility telephone number will access each facility's survey results which occurred January 1, 2005 or after. The survey results describe any deficiencies found during the inspection process over the previous 36-month period. (Note: During the implementation phase of this practice, surveys shown will be for less than a 36-month period.)

Clicking the link labeled “Additional Services” will access a list of extra services and information provided directly by the facility. Please note that not all facilities have provided additional services information for this web site.
