Initial comments:


Based on a Revisit survey on completed on March 14, 2024 it was determined that Armstrong Rehabilitation and Nursing Center was not in compliance with the following requirement of the 28 PA Code, Commonwealth of Pennsylvania Long Term Care Licensure Regulations.









 Plan of Correction:


§ 211.12(f.1)(2) LICENSURE Nursing services. :State only Deficiency.
(2) Effective July 1, 2023, a minimum of 1 nurse aide per 12 residents during the day, 1 nurse aide per 12 residents during the evening, and 1 nurse aide per 20 residents overnight.

Observations:


Based on review of nursing time schedules and staff interview, it was determined that the facility administrative staff failed to provide a minimum of one nursing assistant per twelve residents during the daylight shift for two of five days (3/9/24 and 3/10/24), and on the evening shift on three of five days (3/8/24, 3/9/24 and 3/10/24), and failed to provide a minimum of one nursing assistant per twenty residents on night shift on one of five days (3/12/24).

Findings include:

On the daylight shift:
3/9/24census 7946.30 actual hours49.38 hours required.
3/10/24census 8046.28 actual hours50.00 hours required.

On the evening shift:
3/8/24census 7945.97 actual hours49.38 hours required.
3/9/24census 7941.85 actual hours49.38 hours required.
3/10/24census 8046.17 actual hours50.00 hours required.

On the night shift:
3/10/24census 7922.98 actual hours29.63 hours required.

During an interview on 3/14/24, at 2:45 p.m. the Director of Nursing confirmed the facility administrative staff failed to provide the required minimum nursing assistants on the above listed dates.




 Plan of Correction - To be completed: 04/01/2024

1. The residents had no negative outcome from not meeting a minimum of one nurse aide per twelve residents during the day and /or evening shift.
2. DON/designee will provide the Staffing Coordinator/HR with re-education on the Pennsylvania staffing requirements for ratios.
3. The facility has increased wages, hired additional staff, attend daily corporate labor calls, instituted staffing meetings to track staffing, and added an additional agency to utilize for staffing needs.
4. Staffing coordinator/designee will audit the ratios weekly x 4 weeks and monthly x 3 months.
5. Results of audits will be submitted to QAPI committee for tracking and further recommendations.
