Initial Comments:Based on an unannounced onsite follow up survey completed on 8/14/25, Omni Home Care had corrected the deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a Medicare recertification survey completed on 4/17/25.

The deficiency was cited as a result of a state re-licensure survey completed on 4/17/25.
